Home First is a program of the MA Department of Developmental Services (DDS), Metro Boston Region, which provides enhanced Family Support & Complex Care Management to DDS eligible individuals and their families caring for a Medically Complex Child/Young Adult (3-25 years) who is living at home.
Home First’s specialized case management assists families in navigating medical services, health care benefits, equipment and assistive technology, home nursing, educational services, home and vehicle modifications and public benefits. By integrating these services, HMEA helps families provide the best care for their child in the home.
Staff from the Home First program are personally familiar with many of these challenges, as they have family members supported by DDS and can truly relate to and provide support and understanding to families referred to this program.
Referrals are only made by the local Department of Developmental Services (DDS) Metro Region Family Support Director for the following areas: Newton/South Norfolk, Greater Boston, Charles River West and Middlesex West.
